What is My Health Passport?

The Passport is a collection of knowledge of the person cared for that is provided by themselves and by the family, when cared for at home. It can be added to by professionals, but it is not a formal clinical record.

It does however provide such vital care knowledge as routines, habits and importantly, it hosts six key facts about the person that they like to talk about. That’s a good starting point for any nurse or carer when commcing any care activity.

Who uses it?

It can be used by any one that has been given right of access. Perhaps the day care centre or doctor, even the hospital if they are approved to view ReMe.

In some cases it can be integrated with care planning software, as used for example by a domiciliary or live in care company.
